Brief Facts:The aircraft was being hovered approximately 15 feet above ground level when it over rotated and the right side collided with the ground.

General Description of Damage:The following aircraft damage was observed during our survey: Broken windshield/canopy. Bent forward fuselage structure at lower/forward windshield/canopy attaching area. Extensive impact damage to the main rotor blades. Wrinkle on fuselage structure at upper. Windshield/canopy attach area. Forward tail boom frame tubes bent on left side. Forward tail boom frame tubes bent on right side. Right side aft fuselage and cabin frame bent/wrinkled. Right side fuel tank abraded and crushed, Tank retaining clamps abraded and bent. Right side lower fuselage and floor are bent/wrinkled. Right side landing gear cross tubes bent 180° under the fuselage. Right side skid tube abraded and bent. Tail rotor drive shaft bent/distorted through full length. Tail rotor drive shaft bearings bent/distorted on drive shaft. Main rotor tower support struts are broken at upper attachments. Tail rotor blade tips broken and bent blades. Aft tail boom frame tubes bent/broken. Teardown inspection required for the engine, transmission, and main rotor gearbox.
